Cefuroxime (tab/powd for oral susp: axetil; inj: Na)
Indications/Uses
Pharyngitis, gonorrhea, otitis media. Zocef Tab: tonsillitis, acute bacterial maxillary sinusitis, acute bacterial exacerbations of chronic bronchitis & secondary bacterial infections of acute bronchitis, uncomplicated skin & skin structure infections, uncomplicated UTI, urethral, & endocervical, early Lyme disease. Susp: Bone & joint infections, bronchitis & other lower resp tract infections, meningitis, peritonitis, sinusitis, skin infections, UTI. Prophylaxis for surgical infection. Zocef Hospi Community acquired pneumonia, acute exacerbations of chronic bronchitis, complicated UTI including pyelonephritis, soft-tissue infections eg, cellulitis, erysipelas & wound infections, intra-abdominal infections. Prophylaxis against infection in GI (including oesophageal), orthopedic, CV & gynecological surgery (including caesarean section).
Dosage/Direction for Use
Zocef tab Adult & adolescent ≥13 yr Pharyngitis/tonsillitis 250 mg bid for 10 days. Acute bacterial maxillary sinusitis 250 mg bid for 10 days. Acute bacterial exacerbations of chronic bronchitis 250 or 500 mg bid for 10 days. Secondary bacterial infections of acute bronchitis 250 or 500 mg bid for 5-10 days. Uncomplicated skin & skin structure infections 250 mg bid for 7-10 days. Uncomplicated gonorrhea 1,000 mg as a single dose. Early Lyme disease 500 mg bid for 20 days. Ped patient who can swallow tab whole Acute otitis media 250 mg bid for 10 days. Acute bacterial maxillary sinusitis 250 mg bid for 10 days. Zocef susp Adult Uncomplicated UTI 250 mg bid. Resp tract infections 250-500 mg bid. Childn >3 mth 250 mg bid or 10 mg/kg bid. Max: 250 mg daily. Childn >2 yr Otitis media 250 mg bid or 15 mg/kg bid. Max: 500 mg daily. Zocef Hospi Adult & childn ≥40 kg Community acquired pneumonia & acute exacerbations of chronic bronchitis, soft tissue infections eg, cellulitis, erysipelas & wound infections, intra-abdominal infections 750 mg every 8 hr as IV or IM. Complicated UTI including pyelonephritis 1.5 g every 8 hr as IV or IM. Severe infections 750 mg every 6 hr as IV or 1.5 g every 8 hr as IM. Surgical prophylaxis for GI, gynecological surgery including caesarean section) & orthopaedic operations 1.5 g upon induction of anesth. May be supplemented w/ two 750 mg after 8 & 16 hr as IM. Surgical prophylaxis for CV & oesophageal operations 1.5 g upon induction of anesth followed by 750 mg every 8 hr for a further 24 hr as IM. Infant & toddler >3 wk & childn <40 kg Community acquired pneumonia, complicated UTI including pyelonephritis, soft-tissue infections eg, cellulitis, erysipelas & wound infections, intra-abdominal infections 30-100 mg/kg/day as IV in 3 or 4 divided doses. A dose of 60 mg/kg/day is appropriate for most infections. Infant from birth to 3 wk Community acquired pneumonia, complicated UTI including pyelonephritis, soft-tissue infections eg, cellulitis, erysipelas & wound infections, intra-abdominal infections 30-100 mg/kg/day in 2 or 3 divided doses as IV. Renal impairment CrCl 10-20 mL/min/1.73 m2 750 mg bid, <10 mL/min/1.73 m2 750 mg once daily. Patient on haemodialysis 750 mg dose should be given IV or IM at the end of each dialysis. Patient in renal failure on continuous arteriovenous haemodialysis (CAVH) or high-flux haemofiltration in intensive therapy units 750 mg bid.
Administration
FC tab: May be taken with or without food. Powd for oral susp: Should be taken with food.
Contraindications
Hypersensitivity to cefuroxime or to cephalosporins. History of severe hypersensitivity to any other type of β-lactam antibacterial agent (penicillins, monobactams & carbapenems).
Special Precautions
Zocef Previous hypersensitivity reactions to penicillins or to other type of β-lactam agent. Concurrent treatment potent diuretics eg, furosemide or aminoglycosides. Patients w/ history of colitis. May be associated w/ a fall in prothrombin time. Overgrowth of nonsusceptible organisms. +ve Coombs' test. Slight interference w/ Cu reduction methods (eg, Benedict's, Fehlings, Clinitest). False -ve may occur in the ferricyanide test. Not for intracameral use. Renal impairment. Elderly.
Adverse Reactions
Erythema multiforme, SJS, TEN. Zocef Tab: Anaphylaxis, angioedema, pruritus, rash, serum sickness-like reaction, urticaria; pseudomembranous colitis; hemolytic anemia, leukopenia, pancytopenia, thrombocytopenia, & increased prothrombin time; hepatic impairment including hepatitis & cholestasis, jaundice; seizure; renal dysfunction. Susp: Diarrhea, nausea, vomiting. Zocef Hospi Neutropenia, eosinophilia, decreased Hb conc, transient rise in liver enzymes; inj site reactions which may include pain & thrombophlebitis.
Drug Interactions
Zocef May affect the gut flora leading to lower oestrogen reabsorption & reduced efficacy of combined oral OCs. False -ve result in ferricyanide test. Prolonged excretion & elevated peak serum level w/ probenecid. +ve Coombs' test. Zocef Hospi Strong-acting diuretics (eg, furosemide) or potential nephrotoxic prep (eg, aminoglycosides). Increased INR w/ oral anticoagulants.
